I fully consent to receiving Biodynamic Craniosacral Therapy sessions with Evelyn Olson.

Sacral Sol BCST/Evelyn Olson is a Biodynamic Craniosacral Therapist providing personal care services. Sacral Sol BCST/Evelyn Olson is not a licensed psychologist, psychiatrist, nurse, pediatrician, or medical doctor, and does not diagnose illnesses or injuries, and does not prescribe or administer medication.

I have clearance from my psychologist, psychiatrist, medical doctor etc. to receive Biodynamic Craniosacral Therapy (BCST). I will consult a medical professional prior to adjusting medications and/or treatments.

I understand that it is my responsibility to inform my BCST therapist of any discomfort I am experiencing before, during, or after a session so the appropriate adjustments can be made.

I agree to reschedule my appointment if I am exhibiting signs/symptoms of illness. A BCST session involves maintained touch & close physical proximity over an extended period of time. The risk of illness transmission may be elevated.

I am participating in BCST sessions of my own free will & I release Evelyn Olson/Sacral Sol BCST from any and all liability past, present, and future.

I understand that electronic signing and submission of this document is legally binding.
